General Terms and Conditions
NkwaPlus
1. Digital Remote Care is provided by the medical team of your health facility in collaboration with PharmAccess Ghana, using an app developed by Afrifanom called NkwaPlus.
NkwaPlus is a digital tool to help monitor patients who can measure and monitor their health by themselves remotely using NkwaPlus. This helps patients and their doctors to get an insight into the health of the patient, where the patient is only required to visit the doctor when needed. These general terms and conditions apply to your use of NkwaPlus.
2. We recommend that you read these Terms and Conditions carefully so that you are aware of your rights and obligations when using NkwaPlus. These General Terms and Conditions of NkwaPlus may change from time to time, for example due to changes in laws and regulations. You will be informed about this in a timely manner via a message on the application or by email. You can always consult the most up-to-date terms and conditions via the website (?).
3. This service is not a substitute for emergency care or regular care. Everyone can (continue to) use regular care. You are under no circumstances obliged to use NkwaPlus.
4. You will be periodically requested to fill in your measurement results in the app. The clinical team at your facility will assess the information you provide in the NkwaPlus. If your measurement is above or below a certain threshold, you are requested to reach out to the hospital immediately. Depending on your shared daily measurements, an alert will be sent to the medical team at your hospital when you exceed an agreed threshold. Also, the medical team at your hospital may follow up accordingly, which may include the advice to visit your hospital, urgently, or a notification through the NkwaPlus platform to measure more often (daily) or continue measuring as usual (bi-weekly). If you need emergency care, please take direct action by calling your doctor.
5. NkwaPlus is a service intended for personal use. You may not use the app for any other purposes than for what it is initially intended. You are therefore forbidden to give others access to NkwaPlus. Your healthcare facility reserves the right to adjust and/or extend the response times. Your healthcare facility may also decide at any time to refuse new registrations for NkwaPlus and/or to stop existing participation.
6. You can use the NkwaPlus app if you are 18 years or above, live in Ghana and are a patient registered at your onboarding facility. The coverage area may be expanded later. You need an email address to receive our emails and a suitable mobile device (phone, tablet or iPad). The device must have Android version 5 or higher OR Apple version iOS12 or higher. You must also agree to the terms of use in the app (.........................).
8. The data you enter in the app is read by the clinician at your facility. This team consists of healthcare providers, and their supporting team.
9. Advice via NkwaPlus is given based on the information you provide. Your doctor is authorized to use the data from the app in combination with your regular patient file. You are responsible for providing information in the app to the best of your knowledge. We expect you to provide accurate and complete health information. To be able to assist you best remotely, it is important that you are willing and capable to measure your levels periodically and to send them in via the app. Please try to be reachable by phone as much as possible.
10. You are responsible for providing an internet connection for your mobile device and for the costs thereof. Your facility is not responsible or liable for malfunctions in the availability of internet. We cannot guarantee fully uninterrupted access to the app.
11. Your facility's clinical team can (together with you) decide to close your access to the app. This might be because you requested to close your access or in case of your inability to provide required information. The team will then initiate the procedure to delete your account. You can also always log out yourself and uninstall the app from your device.
12. In case you have a complaint, questions or advice, please contact your facility via this number(03002735261). As soon as there is room for this, your complaint will be processed.
